Yes! Heartland Behavioral Homes LLC is an approved provider under Florida’s Medicaid Waiver Program. We proudly serve individuals enrolled in the iBudget Waiver, delivering state-approved personal support services that help individuals with disabilities live more independently in their homes and communities.
The admission process at Heartland Behavioral Homes typically involves an initial call or referral assessment to determine the individual's needs and eligibility. This is followed up by two separate meeting, one without the individual present and if both parties feel that it's a great fit we move forward to the meet and greet with the individual present. Our meeting will discuss the individual's goals and support needs. Finally, the individual and their support network will work with us to develop a care plan and transition plan.
Our staff are highly trained and experienced in working with individuals with disabilities. We provide ongoing training and support to ensure that our staff stay in compliant with APD, Florida Medicaid and are equipped to meet the unique needs of our residents.
Getting started is easy. Simply contact us by phone or email to schedule a consultation. If you're already enrolled in the Medicaid Waiver Program, we’ll coordinate with your support coordinator. If not, we can walk you through how to apply for eligibility through Florida's APD.
Individuals of all ages with intellectual or developmental disabilities may qualify for Florida’s iBudget Medicaid Waiver. To be eligible for services through Angels on Earth, clients must be approved by the Agency for Persons with Disabilities (APD) and enrolled in the waiver. We can help guide families through the process.

Q1: What’s the difference between Personal Supports and Companion Services?
A: Personal Supports involve direct assistance with daily living activities like bathing, dressing, or preparing meals. Companion Services, on the other hand, focus more on supervision and social interaction—not hands-on help.
Q2: Who provides the personal support care?
A: All support is delivered by trained, APD-compliant direct support professionals (DSPs) who are background-checked and certified in working with adults with developmental disabilities.
Q3: Can support staff come to my family member’s home?
A: Yes. Services are typically provided in the individual’s own home, family home, or licensed residential setting.
Q4: Is this service available full-time?
A: Yes. Hours are flexible and can be scheduled according to the person’s approved support plan and funding availability.
Q5: Do you accept Medicaid Waiver funding?
A: Yes. We are a Medicaid Waiver-approved provider through the Agency for Persons with Disabilities (APD) in Florida.
Q1: What do companion staff actually do during a typical visit?
A: Companions may talk, play games, assist with community outings, attend events, or simply provide supervision and emotional support based on the person’s needs and interests.
Q2: Is this service for individuals who need physical help too?
A: No. Companion Services are intended for individuals who do not require physical assistance but still benefit from supervision, social interaction, or structured companionship.
Q3: Can companions take individuals out into the community?
A: Yes. Outings to local parks, stores, events, or recreational activities are encouraged when appropriate and based on the individual’s plan.
Q4: Is transportation included?
A: Transportation may be provided as part of the approved services, such as taking the individual to appointments or community activities.
Q5: How do I know if my loved one qualifies?
A: If they are eligible for APD services and need non-medical supervision or companionship, they may qualify. We can help you determine eligibility during a consultation.
Q1: What types of jobs do you help people find?
A: Jobs vary based on each person’s interests and strengths. We’ve supported placements in retail, food service, janitorial work, landscaping, clerical roles, and more.
Q2: Does someone stay with the individual at their job?
A: Initially, yes. A job coach provides on-site support to help them learn tasks and adjust. Over time, as independence builds, our presence gradually fades—but follow-up support remains available.
Q3: Do you only help find jobs, or also help keep them?
A: We do both. Our Supported Employment services include job development, training, on-the-job coaching, and ongoing support to ensure long-term success.
Q4: Is this only for people with Medicaid Waiver funding?
A: Primarily yes, but we may be able to work with private pay clients or coordinate with Vocational Rehabilitation depending on your situation.
Q5: Can you help someone who has never worked before?
A: Absolutely. We support individuals with no job experience, focusing on building job readiness and matching them with employers who understand and value diversity.
